Short answer

STIs don’t go away without treatment in any plannable sense: signs fade while organisms persist, latency deceives, and complications accrue silently.

The “wait and see” strategy has a documented casualty list. While prompt treatment closes nearly every one of these stories cleanly.

What fades vs what persists

Fades: chancres healing spontaneously, rashes resolving, warts sometimes regressing, discharge fluctuating, outbreak episodes ending.

Persists: treponemes progressing toward organ damage, gonococci and chlamydia ascending toward reproductive harm, herpes latent with ongoing shedding, HIV replicating, high-risk HPV persisting toward precancer.

Fading symptoms mark disease phases changing. Never infection ending.

This fluctuation pattern is explained in Why STI Symptoms May Come and Go.

The cost of waiting, stated plainly

PID and infertility, ectopic pregnancy, congenital infections in future pregnancies, neurosyphilis and tertiary damage, HIV progression, HPV cancers, and months of unwitting transmission to partners.

Each has the same structure: a curable-or-manageable infection plus delay equals permanent harm. The curable/manageable distinction itself is explained in Common STI Treatment Myths Explained.

Why waiting appeals (and fails)

Stigma, cost fears, hope, misread remission, and distrust. Each solvable through confidential low-cost clinics that treat routinely.

Shame fades within one normalizing clinic visit; payment worries resolve at sliding-scale sites; hope gets replaced by same-day answers; remission gets reinterpreted the moment a clinician explains latency; distrust softens when results arrive with a plan.

Cost questions are answered in Can You Get STI Testing Without Insurance? and How Much Does STI Testing Cost?.

Barriers are real; letting them decide is costlier than any visit.

When to seek care

Now for any untreated possible STI regardless of current symptoms; urgently for pregnancy, severe symptoms, or exposures needing PEP; on schedule for screening that prevents the question entirely.
