Short answer

Treatment is prevention for HIV. Literally.

When antiretroviral therapy suppresses viral load to undetectable and keeps it there, sexual HIV transmission stops entirely (U=U).

That single fact reorganizes everything: every linkage-to-care success, every adherence support, every viral-load check is simultaneously individual healthcare and community prevention.

The science is settled by large partner studies; the remaining work is implementation.

The U=U evidence base

The PARTNER 1 and PARTNER 2 studies followed serodiscordant heterosexual and gay couples through tens of thousands of condomless sex acts with suppressed viral loads. Zero linked transmissions.

Opposites Attract replicated the finding in MSM couples.

Together they establish that sustained undetectable viral load eliminates sexual transmission risk, a result now endorsed by CDC and hundreds of health organizations. “Undetectable” means below 200 copies/mL sustained over time with ongoing monitoring. A precise status, not a feeling.

Sustaining suppression in real life

Suppression is achieved by daily adherence (pill organizers, phone alarms, pharmacy auto-refills, long-acting injectables where eligible), protected by viral-load monitoring cadence (detecting blips before they become rebounds), and defended against interruptions (travel supplies, refill timing, side-effect management through switches not stops).

Resistance management covers breakthroughs: genotype testing identifies escaped mutations, regimens adjust, suppression re-establishes.

Mental-health and substance-use supports protect adherence indirectly but powerfully. The most common suppression failures are life failures, not drug failures.

Layered context that U=U doesn’t replace

Condoms still matter for other STIs and as backup during viremic episodes.

Partner PrEP provides belt-and-suspenders protection during early treatment before suppression confirms.

STI screening continues. Genital ulcers amplify any residual transmission risk. Harm reduction covers drug-use transmission routes ART doesn’t touch.

U=U is the centerpiece of modern HIV prevention, not the whole building.

When to seek care

Immediately at diagnosis (rapid ART start. Same-day protocols exist at many clinics), for adherence support services before gaps become rebounds, with any detectable viral load (resistance workup, not shame), and for partner PrEP navigation protecting partners during the journey to suppression.
